What It Is
Individual and family dental coverage with options vision add-ons

Immediate Dental and Vision Benefits
  1. Day-one dental benefits (6-month waiting period for Major under Copay Plan 1)
  2. Optional vision add-ons

Dental Plan Levels
  1. Coinsurance plans (Value, Plus, Prime) - percentage-based coverage that increases over time
  2. Copay Plans (Copay 1, Copay 2) - fixed-dollar payments per visit or procedure, with no deductibles and predictable out-of-pocket costs

Dental Benefits
Coverage includes:
  1. Preventative Care -exams, cleanings, fluoride, x-rays
  2. Basic: fillings, extractions, emergency treatment, repairs
  3. Major: crown, oral surgery endodontics, etc.

Large National PPO Network
Plans use the Aetna Dental Administrator Network. Out-of-network care is covered but at lower benefit levels.

Vision Benefits
Uses Avesis Vision Network
  1. Level 1 Plan : $15 exam copay, $130 frame/contact max (every 24 months)
  2. Level 2 Plan: $10 exam copay, $200 frame contact max (every 12 months)
Includes lens and coating benefit with discounts or full coverage

Increasing Benefits
Coverage levels and annual maximum increase each year for the first three years:
  1. Year 1: 60% coverage (basic services), $1,000 max
  2. Year 2: 80% coverage, $1,500 - $2,500 max
  3. Year 3: 80% coverage, $3,000 max
All have a $50 individual/$150 family annual deductible - waived for routine cleaning exams and x-rays

Orthodontic Coverage
The Prime plans includes orthodontic coverage for children under 19, starting at 15% in the first year and rising to 50% in year two, with a $1,000 annual maxiumu.

Eligibility & Renewability
  1. Ages 0-64 for dental; child-only plans available
  2. Renewable up to age 65; premiums may adjust with notice

What’s Not Covered
  1. Cosmetic dentistry or teeth whitening
  2. Implants, orthodontics (unless included)
  3. Crowns/dentures replaced within 10 years
  4. Family or non-licensed provider dental care
  5. Experimental, non-medically necessary procedures
  6. Vision exclusions (various)
