What It Is
Fixed-benefit dental plan that pays set dollar amounts for covered dental services.

Predictable Cash Payments
  1. Plans have a know, fixed benefit amount
  2. Amounts pare paid directly to the client
Dental Plan Levels
  1. Basic
  2. Intermediate
  3. Plus
Dental Benefits
Coverage includes:
  1. Preventive Care -exams, cleanings, fluoride, x-rays
  2. Basic: fillings, extractions, emergency treatment, repairs
  3. Major: crown, oral surgery endodontics, etc. (Plus Plan only. 180 waiting period in most states)
Preventive & Basic Services Start Day One
  1. All plans have preventive and basic services with no waiting periods
  2. Preventive services do not count toward the annual maximum
Annual Benefit Maximums
  1. Basic: $500
  2. Intermediate: $1,000
  3. Plus: $1,500
Once the maximum is reached, additional services are member-paid

Increasing Benefits
For Basic Services:
  1. Year 1: 50% of listed benefit
  2. Year 2+: 100 of listed benefit
Optional Network Savings Card
Uses the Careington Maximum Care Network
  1. Save an average of 45% on dental services
  2. Access vision discounts
  3. Savings apply on top of cash benefits
Eligibility & Renewability
  1. Primary insured & spouse: Ages 14-94
  2. Dependent children: Birth through age 24
What’s Not Covered
  1. Cosmetic dentistry or teeth whitening
  2. Implants, orthodontics (unless included)
  3. Family or non-licensed provider dental care
  4. Experimental, non-medically necessary procedures
