What It Does
Pays a fixed amount directly to the member based on the benefit levels of the plan chosen for covered hospital stays. The client can use these fund any way they wish.

Base (A-C) and Max (D-F)

Eligibility and Renewability
Ages 0-64

Benefit Amounts
  1. Admission: $1,500 - $5,000 (depending on plan)
  2. ICU: $1,000/day
  3. Daily confinement: $150/day up to 10-31 days
  4. Ground ambulance: $250/trip
  5. Air Ambulance: $25,000/trip
Outpatient Benefits (Max Plans only)
  1. Office visits: $75/visit, 2 per year
  2. Urgent care: $75/visit, 2 per year
  3. Radiology and lab: $50/test, 2 per year
Waiting Periods
Immediate coverage from day one for most benefits

No Networks
Members can go anywhere for treatment with no provider restrictions. Benefits pay the same.

Pre-Existing Condition Limitations
Excludes benefits for any injury or sickness linked to a pre-existing condition during the first 12 months. Full exclusion list includes:
  1. Mental illness, substance abuse
  2. Pregnancy (pre-existing)
  3. Hazardous sports
  4. Dental/vision care
  5. Weight loss surgery
  6. Gender-related treatments
