What It Does

Provides cash benefits directly to you after an accident, critical illness or hospitalization and is guaranteed issue

Modular, Customizable Coverage

1. All plans have Accident Medical Expense (AME) coverage
2. Cancer and Heart/Stroke Rider available
3. Sickness Hospitalization Rider available
   

Benefit Amounts

1. $2,500 - $17,500
2. Benefits increase annually with disappearing deductibles
3. Higher payouts for certain coverages in year 2 and 3
   

AME coverage

1. Pays covered medical expenses up to selected amount
2. No waiting period or network restrictions
3. Includes accidental death & dismemberment benefits
4. Decreasing deductibles
5. Can be purchased alone
   

Cancer and Heart/Stroke Rider

1. Lump-sum, cash benefit on first-ever covered diagnosis
2. 90-day wait period for cancer/30 days for heart/stroke
3. 12 month pre-existing limitation
   

Sickness Hospitalization Rider

1. Lump-sum benefits on first covered day of hospitalization due to sickness
2. No deductible and no network restrictions
3. One benefit per year/two per family
4. 30-day waiting period with a 12-month preexisting condition limitation
   

Guaranteed Issue & No Network Limits

1. All coverage types are guaranteed issue
2. Members can visit any doctor or hospital without preauthorization
   

Eligibility & Renewability

1. Adults ages 18-64 and dependents up to age 25
2. AME renewable to age 75, riders renewable to age 65
   

What’s Not Covered

1. Cosmetic or experimental services
2. War, terrorism, or hazardous activities
3. Substance use or self-inflicted injury
4. Maternity, fertility, or weight-loss treatment
5. Non-emergency services outside the U.S.
6. Mental health or behavioral conditions
