What It Is
DentalWise is a standalone dental insurance plan for individuals and families, administered by Dental Benefit Providers.
No Waiting Periods
No waiting periods for preventive, basic and most major services. EXCEPTION: Implants do have a 12-month waiting period.
Dental Plan Levels
All have a $100 annual deductible - waived for routine cleaning exams and x-rays
Dental Benefits
Coverage includes:
- Preventative Care - 100% covered (2 cleanings & exams per year)
- Basic - fillings, extractions, anesthesia
- Major - Crowns, bridges, dentures, root canals (1000 & 2000 only)
- Implants - 50% covered (Plan 2000 only) with $1,500 lifetime max.
Large National PPO Network
Members can visit any dentist, but using National Options PPE 30 network provides lower costs and removed claim paperwork.
No-Claim Convenience
When members use in-network providers for any services, there are no claim forms - providers handle billing directly.
Eligibility & Renewability
Adults ages 18-99 and dependents under 26 (extends per state) with renewable coverage.
