Allowance System
The plan pays a set maximum amount (“allowance”) for eyewear and contact purchases. If the total cost is below the allowance, members pay nothing more; if it exceeds the limit, they cover the difference.

VisionWise uses the UnitedHealthcare Vision Network, which includes local optometrists, ophthalmologists, and major retail vision centers. You can search providers at myuhcvision.com
No-Claim Convenience
When members use in-network providers for any services, there are no claim forms - providers handle billing directly. For out-of-network visits, members pay upfront and submit receipts for reimbursement.
Renewability
Coverage is renewable as long as premiums are paid. UnitedHealthcare can adjust premiums with at least 31 days’ notice, based on policy class.
What’s Not Covered
- Non-prescription or optional lens extras
- Lost/broken eyewear outside normal intervals
- Experimental or surgical eye treatments (i.e. LASIK)
- Medical/surgical eye care or treatments by family members
- Services provided before or after policy dates
