What it is

DentalWise Max combines Dental, Vision, and Hearing (DVH) benefits into one plan for individuals and families. It’s designed to promote overall wellness with convenient, bundles coverage. 

Immediate Dental and Vision Benefits

Most dental and vision services have no waiting period, allowing members to use benefits right away.

Dental Plan Levels

1. Plan 1000 - Up to $1,000 annual maximum
2. Plan 2000 - up to $2,000 annual maximum
3. Plan 3000 - up to $3,000 annual maximum
   

All have a $100 annual deductible - waived for routine cleaning exams and x-rays

Dental Benefits

1. Preventative Care - 100% covered (2 cleanings & exams per year)
2. Basic - fillings, extractions, anesthesia
3. Major - Crowns, bridges, dentures, root canals
   
4. Implants - 50% covered (Plans 2000 and 3000 only) with $1,500 lifetime max.
   

Large National PPO Network

Members can visit any dentist, but using National Options PPE 30 network provides lower costs and removed claim paperwork.

Vision Benefits

Includes an annual eye exam and eyewear allowance:

1. $0 exam copay (in-network)
2. $150 frame of contact lens allowance
3. Coverage for standard lenses with $10 copay
   

Administered by Spectera Inc. (UnivedHealthcare Vision)

Hearing Benefits

Provided through UnivedHealthcare hearing

1. 100% coverage for annual hearing exam
2. $750 allowance every 2 years for prescription or OTC hearing aids when purchased through network providers
   

No-Claim Convenience

When members use in-network providers for any services, there are no claim forms - providers handle billing directly.

Eligibility & Renewability

Adults ages 18-99 and dependents under 26 (extends per state) with renewable coverage.
