As part of our contracting process for Select Health, we will need you to complete the following form. You will be required to attach the following:
- Select Health Form: https://zfrmz.com/lrnKStvPzF49vWSf7PVO
- A copy of your Resident License
- A copy of your UT License
- A copy of your E&O Policy
- A copy of your FFM Cert
Once we have reviewed this form, we will send you our Associate Agent Agreement. This will set you up in our system, acknowledging you are only working with Compass Insurance Advisors for Select Health. Additionally, to set you up in our system to receive commissions, we will need the following,
